INFORMATION FOR PATIENTS

Welcome to my practice.  Your comfort is important, please note some suggestions below.

On the day of your appointment

Please wear comfortable clothes or bring shorts and a top to change into. Preferably loose fitting and cotton. That way I can both work on you and have you move around as we need.  

Please refrain from heavily scented products and or make-up. 

 

New Patient Questionnaire  (download .pdf)

You can fill out the questionnaire and return it via email. Or you can print it, fill it out at home and bring it with you on your first appointment. Or you can arrive to your first appointment 15 minutes early and fill it out in the office waiting area.

 

Time Considerations

Intake sessions are scheduled for 90 minutes, and ongoing sessions are 50 minutes. Please arrive on time. I schedule on the hour and if you are late I will not be able to make up the time for you.

 

Cancellation Policy

Please contact me if you need to change your appointment time. I will accommodate you as best I can. I request 48 hour notice for cancelling an appointment, and within 24 hours you will be responsible for the fee.  I will send a pre-appointment reminder text on the evening before we are scheduled.

 

Fees and Sliding Scale

My standard fee is $140/hour.  Please let me know if economic hardship is keeping you from getting the care you need. I will do what I can to work with you.

 

Privacy

I will keep your name and anything shared confidential.

 

Insurance

Please understand I cannot bill insurance companies directly.  I can give you a receipt for reimbursement, and if there is another way I can help please let me know.
