INFORMATION FOR PATIENTS
On the day of your appointment
Please wear comfortable clothes or bring shorts and a top to change into. Preferably loose fitting and cotton. That way I can both work on you and have you move around as we need.
Please refrain from heavily scented products and or make-up.

You can fill out the questionnaire and return it via email. Or you can print it, fill it out at home and bring it with you on your first appointment. Or you can arrive to your first appointment 15 minutes early and fill it out in the office waiting area.
Intake sessions are scheduled for 90 minutes, and ongoing sessions are 50 minutes. Please arrive on time. I schedule on the hour and if you are late I will not be able to make up the time for you.
Please contact me if you need to change your appointment time. I will accommodate you as best I can. I request 48 hour notice for cancelling an appointment, and within 24 hours you will be responsible for the fee. I do not make pre-appointment reminder calls or texts.
Please let me know if economic hardship is keeping you from getting the care you need. I will do what I can to work with you.
I will keep your name and anything shared confidential.
Please understand I cannot bill insurance companies directly. I can give you a receipt for reimbursement, and if there is another way I can help please let me know.
